Do you offer after-hours care?

If you have an emergency, call 911 immediately.

If you have a non-emergency urgent need that cannot wait until office-hours, you are welcome to call your community health center. A medical provider will return your call and work with you to decide if you need to be treated at home, be seen at the health center during business hours, or go to the hospital.

Do you offer lab services?

Our community health centers offer some lab tests in-house. Other tests are sent to NorDx for processing at their Scarborough, Maine, facility. In those cases, you should expect a bill for work done by NorDx. Sometimes, NorDx offers a 20% discount if you pay in cash at the time you receive lab services at one of our health centers. Please inquire about this at the reception desk.

If test results come back normal, you will receive a call from your primary care provider or a nurse to discuss your results, whether normal or abnormal, and next steps.

What services are available on the Patient Portal?

We heard your feedback about your experience with the patient portal. We are excited to announce that we are working to bring you a new and improved way to access your medical information. As a part of this upgrade, the current patient portal will become unavailable starting Friday, March 1. All portal services will be inaccessible until the new system launches. We commit to keeping you informed as we get closer to launching the new patient portal. We apologize for any inconvenience you may experience during this downtime. Rest assured, all of your existing information will be transferred into our new patient portal. Please reach out to your health center with any questions. We thank you for your patience.

How do I get a referral to a specialist?

Your primary care provider is responsible for referrals. Please call your local health center or log in to your Patient Portal if you think you need a referral.

Am I eligible to be a patient at HealthReach?

Yes! Everyone is welcome at HealthReach. Please call your neighborhood health center to make an appointment, and let them know you’re a new patient.

Do you offer treatment or other resources to help with my addiction?
All of our health center locations offer treatment for addictions. This service is also conveniently available through telehealth, so everyone may easily access help.
How do I schedule an appointment?

It’s easy to make an appointment with HealthReach. You may log into the Patient Portal at any time to schedule an appointment, or you can call your local health center. In many cases, same-day appointments are available, including for behavioral health concerns.

What is a Patient-Centered Medical Home (PCMH)?

HealthReach is a patient-centered medical home. This means you have an integrated care team working together to help you. We listen to you and partner with you to meet your healthcare needs. Your care team knows you and keeps track of your health history. We make sure you understand your medical issues and how to deal with them. We help coordinate your care if you need outside tests or specialists, and we make sure you have the support you need to get the care you need. By working together, we make healthcare convenient and easy for you.

We are proud to be recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home.

How do I refill a prescription?

To refill your medicine, log in to your Patient Portal or call your local health center and ask for a refill. Please allow 72 hours for us to process your request.

How can I afford healthcare services at HealthReach?

At HealthReach, we believe everyone deserves the healthcare they need, no matter what you can pay. We offer complete care at a reasonable cost — and we help make sure it’s affordable for you.

We constantly strive to keep patient costs down while maintaining our great quality of care. In addition, we offer an Affordable Care Program (also known as a “sliding fee scale”). Our Connectors are happy to assist you in getting additional support — including health insurance through the Maine Health Insurance Marketplace — and connecting with other support programs. Do you offer behavioral healthcare or counseling services?

Yes. We treat the whole you — including mental health. Our health centers offer behavioral health services and counseling, and many offer same-day appointments. Please call your local health center to make an appointment.

I don’t have health insurance. Can I be seen at HealthReach?

Yes! Everyone is welcome at HealthReach, no matter what you can pay. If you don’t have insurance, you may qualify for our Affordable Care Program.
