Your care team includes a primary care provider (doctor, physician assistant, or family nurse practitioner), care manager, connector, and a licensed clinical social worker. We work together and partner with local support services to give you and your family easy, affordable access to complete care right in your own community. Healthcare Services
Affordable Care Program
HealthReach offers an Affordable Care Program that reduces your care costs based on your income and family size. If you are eligible, this program can reduce the cost of both dental and health center services.
There are many programs that help Maine people with the high cost of health care and medications, but figuring out program eligibility rules and tackling the enrollment processes can be overwhelming. Through the HealthReach Connector Program, patients of any HealthReach Health Center can get assistance with determining eligibility for benefits and submitting applications.
After-Hours Care
If your health center is not open, you can call your health center to reach our after-hours, on-call team. They can discuss your symptoms and treatment options with you. They can give you advice about how to feel better at home or help you connect with the right level of care in the community.
Care for Acute/Chronic Illness
Acute and chronic illness management can be difficult. Our clinicians provide care that will help you best manage your illness and pain — so you can feel better and live your healthiest.

Counseling
DOT Physicals
Department of Transportation (DOT) Physical exams are required to obtain a Commercial Driver’s License (CDL). We can typically offer this service to you at any of our local health centers — please call to confirm and set up your appointment.
Family Planning Services
Family planning involves birth control counseling and prescriptions, as well as general support services — available at all our health centers.
Immunizations & Vaccinations
Vaccines can help you get immunity against certain germs, keeping you — and often the people around you — safer, healthier, and out of the hospital. All our health centers provide vaccines, including for COVID-19 and seasonal flu. Education and counseling are also available to any patient looking for more information.
Joint Injections
Joint injections are medicines that can assist in recovery from injury and can help reduce your pain caused by arthritis, tendonitis, or other issues. Joint injections can be performed at many of our local health centers — please call to confirm and set up your appointment.
Lab Services
“Labs” or “tests” involve collecting a sample (blood, saliva, etc.) from your body so that specialists can determine if disease is indicated. Lab tests help our clinicians diagnose various medical conditions and can be the first step toward creating your individualized care plan.
All our health centers offer many lab tests in-house. For some of those tests that we do not perform in-house, we can conveniently collect your sample and forward it to our lab partner NorDx for offsite testing.

Men’s Health
We offer men’s healthcare at all our health centers — including screenings for certain cancer types, reproductive healthcare, and other important men’s health services.
Minor Procedures (i.e., wart removal)
Osteopathic Manipulation
Osteopathic manipulation is a practice that involves physically maneuvering the patient’s muscles and joints to diagnose and treat physical conditions.
Pediatric Services
General pediatric services for children — including child well checks, recommended vaccinations, and sick visits — are offered at all our health centers.

Patient Portal
A new patient portal is coming, Fall 2024! We heard your feedback about your experience with the patient portal. We are excited to announce that we are working to bring you a new and improved way to access your medical information. As a part of this upgrade, the current patient portal will become unavailable starting Friday, March 1. All portal services will be inaccessible until the new system launches. We commit to keeping you informed as we get closer to launching the new patient portal. We apologize for any inconvenience you may experience during this downtime. Rest assured, all of your existing information will be transferred into our new patient portal. Please reach out to your health center with any questions. We thank you for your patience.

Our mental health professionals can prescribe, adjust, and manage your psychiatric medication to get you the help and support you need to keep your life running smoothly.

Substance Use Disorder Treatment
Triage Department
Call your Health Center and the Triage Nurse can discuss your symptoms and treatment options with you. The Triage Nurse can give you advice about how to feel better at home, consult with a HealthReach clinician, or help you connect with the right level of care in the community – even after-hours.
Women’s Health
We offer women’s healthcare at all our health centers — including regular Pap smears, and other important women’s health services.

Cleanings
Crowns and Bridges
Crowns and bridges permanently replace a missing portion or an entire tooth, or “bridge” the gap between two separated teeth. While crowns and bridges serve different technical purposes, our team of skilled dental clinicians are here to determine which is the best option for you.
Education
Knowing more can help you to protect your oral health and wellbeing. Our dental clinicians provide comprehensive dental education, and are available to answer your questions.

General Dentistry
Protecting your oral health can help you to enjoy eating and prevent or reduce pain. Our dental clinicians offer a variety of general dentistry services — including education, appointment scheduling, answering your questions, and other services — that will help you to maximize your oral health.

Preventative Care
Our dental clinicians review your dental health history and perform oral exams to determine what you may need to watch for, what care you may need to get, and/or how to stay healthy.
Referrals
If you need more complex treatment than we can provide — such as dental or oral surgery — we are happy to refer you to other clinicians that can meet your needs.
Restoration
Restoration means “bringing back” function and health to your teeth. Our dental clinicians will work with you to create a plan to improve your oral health and then keep your teeth healthy.
Sealants
Studies have shown that sealants can prevent an average of 8 out of 10 of cavities for 2 years, and 5 out of 10 cavities up to 4 years after they are applied. The U.S. Center for Disease Control recommends sealants on first molars for children starting at age 6, and on second molars starting at age 12.
