MEDICAL ASSISTANT or LPN

HealthReach Community Health Centers is seeking a MEDICAL ASSISTANT/LPN to support a Psychiatric Mental Health Nurse Practitioner who sees patients on a rotating basis at 2 health centers. Responsibilities include triage, laboratory functions, recording medical information, direct patient care and education, processing mental health referrals, supporting the behavioral health staff and electronic medical record documentation. The ability to multi-task, work in a fast-paced environment, provide quality patient care, and be committed to the team is essential.
